Includes:

• GI Map Test (stool study collection kit)
• One 30 minute telehealth (virtual) appointment where your individual results will be discussed with a licensed medical provider

Results take about 4 weeks to come back in from the date you mail in the collected specimen.

*GI-MAP Test Package Expiration: Once the GI-MAP test is completed and results are available, they should be reviewed as soon as possible to ensure recommendations are based on the most current information, as gut health, microbiome composition, and other health factors may change over time. Results that are more than three (3) months old may no longer accurately reflect your current health status and may require a new test. All packages expire one (1) year from the date of purchase, regardless of use.

Customers are responsible for following through and attending their lab review consultation appointment.

Online Services User Agreement and Terms of Use

1. Overview: This website is operated by the Practice (“Company,” “we,” “us,” or “our”). By accessing this site or purchasing products or services, including dietary supplements, laboratory testing, and consultation services, you agree to be bound by these Terms of Use (“Terms”).

2. No Medical Advice (Website Content): All information provided on this website, including product descriptions, educational materials, and laboratory testing information, is for informational purposes only and is not intended as medical advice, diagnosis, or treatment. Dietary supplements sold through this website have not been evaluated by the U.S. Food and Drug Administration and are not intended to diagnose, treat, cure, or prevent any disease. Store Purchases vs. Medical Services: The Company offers both (a) ecommerce products and services, including dietary supplements and certain laboratory testing options (“Store Purchases”), and (b) clinical services, including consultation and laboratory review appointments (“Medical Services”). Store Purchases are separate from Medical Services. Purchasing supplements or laboratory tests does not establish a provider–patient relationship and does not include individualized medical advice, diagnosis, or treatment. Medical Services are provided only through a scheduled consultation with a qualified provider and are subject to additional clinical consents and applicable laws and regulations.

4. Laboratory Testing Services: Laboratory testing services offered through this website may be fulfilled by third-party providers. By purchasing laboratory testing services, you acknowledge and agree that: The Company does not perform laboratory testing; Testing may require authorization by an independent licensed provider where required by law ; Laboratory services may not be available in all jurisdictions; Results are for informational purposes only and require interpretation by a qualified healthcare provider  

5. Telehealth and Consultation Services: The Company may offer consultation services, including laboratory review appointments. A provider–patient relationship is established only after: You schedule a consultation; A provider accepts the appointment; You complete all required intake and consent documentation, and; You engage in the scheduled consultation with the medical provider. Medical Services are based on the information available at the time of the consultation and may be limited in scope. No guarantees are made regarding outcomes, recommendations, or results. These services are not intended for emergency care. If you are experiencing a medical emergency, call 911 or seek immediate medical attention.

6. State Restrictions and Eligibility: Medical Services are provided only to individuals physically located in states where the Company’s providers are licensed to practice at the time of the consultation. By scheduling a consultation, you represent and warrant that you will be physically located in an eligible state at the time services are rendered. The Company reserves the right to decline or cancel Medical Services if you are located in a jurisdiction where its providers are not authorized to practice. Store Purchases and laboratory testing services are subject to varying state and federal regulations, and availability may differ by location. Genetic testing services, where offered, may be available to customers regardless of geographic location, including outside of the United States, subject to applicable laws and third-party provider requirements.

7. Lab Review Disclaimer: Laboratory review consultations are intended to provide general clinical insight based on available test results and the information you provide. These consultations are limited in scope and are not a substitute for comprehensive medical care. Laboratory results should be interpreted in conjunction with a full clinical evaluation by a qualified healthcare provider. You should not rely solely on laboratory review consultations to make medical decisions.

It is the sole responsibility of the customer to schedule and complete the lab interpretation appointment. Hope for Healing is not responsible for ongoing follow-up or outreach for clients who have not established medical care through a scheduled appointment with Hope for Healing.

8. Orders, Payments, and Refunds: All purchases, including supplements, laboratory testing services, and consultations, are subject to availability and acceptance. Refunds are governed by the Company’s Refund Policy. Certain items, including opened supplements and processed laboratory orders, are non-refundable due to third-party processing and safety standards. Consultation fees are non -refundable after services are provided or for late cancellations outside the 72 hour required notice period.  All other refunds are subject to the clinic's refund policy.

9. Shipping and Fulfillment: Shipping times are estimates only and are not guaranteed. The Company is not responsible for delays caused by shipping carriers or third-party fulfillment providers.

10. Third-Party Services and Products: The Company may offer or facilitate access to products or services provided by third parties, including laboratories and supplement manufacturers. The Company does not control and is not responsible for the quality, accuracy, performance, or availability of third-party products or services.

11. Limitation of Liability: To the fullest extent permitted by law, the Company shall not be liable for any indirect, incidental, consequential, or special damages arising from your use of this website, products, or services. All products and services are provided on an “as is” and “as available” basis. Your use of this website and any products or services is at your own risk.

12. Indemnification: You agree to indemnify and hold harmless the Company from any claims, damages, or expenses arising out of your use or misuse of the website, products, or services, or your violation of these Terms.

13. Governing Law: These Terms shall be governed by and construed in accordance with the laws of the State of Texas.

14. Modifications to Terms: The Company reserves the right to update or modify these Terms at any time. Continued use of the website following any changes constitutes acceptance of those changes.
